- 2 Completed Recommendation Forms in a Sealed Envelope
- Certified copy of birth certificate (for Filipinos)
- Parent's marriage contract
- 2 pcs. 2x2 ID photo (Applicants must wear a white collared shirt, on white background)
- Certified copies of school records (for the last 2 years)
- Photocopy of First Communion certificate
- Copy of passport pages (for non-Filipino applicants)
- Copy of ACR I-Card (for non-Filipinos)
- Special needs report
- Documents supporting guardianship
