1.  Fill up Student Permanent Record (SPR) & Info Sheet
2. Verification of Required Documents (if available):

•  HK Certificate
• Certificate of Indigency

3. If HK Scholarship awardee, validation of award.
4. Encoding of SPR
5. Verification of Required Documents (if available):

•  HK Certificate
• Certificate of Indigency

6. If HK Scholarship awardee, internal tagging.
7.  (Required) Applicant pays for PhP 500 minimum downpayment.
8. Issuance of Official Receipt (OR) of downpayment.
9. ​Issuance of Reservation/ Welcome Letter
10. Present Official Receipt and Confirmation Letter
11. Issuance of other information/ instruction re: other enrollment details

4. Downpayment

• Non-scholars: pay an additional P2,000.00 downpayment to get ID & 1 set of uniform
• For scholars: can pay P750.00 for ID or P1,500 for both ID & 1 set of uniform

• Non-scholars: pay P2,500.00 down payment to get ID & 1 set of uniform
• For HK scholars: can pay P750.00 for ID or P1,500 for both ID & 1 set of uniform

1. Printing of Official Registration Form (ORF) or Temporary Registration Form (TRF) in case the schedule is not yet ready.

a. If enrollee has Slot Reservation:
Non-scholars: pay an additional P2,000.00 downpayment to get ID & 1 set of uniform
For scholars: can pay P750.00 for ID or P1,500 for both ID & 1 set of uniform


b. If enrollee has NO Slot Reservation:
Non-scholars: pay P2,500.00 down payment to get ID & 1 set of uniform
For HK scholars: can pay P750.00 for ID or P1,500 for both ID & 1 set of uniform

Go to the Dean's Office (See Dean or Program Head) for advising and subject crediting.
