    OLFU has five (5) campuses namely Valenzuela City; Quezon City; Antipolo City; San Fernando, Pampanga; and Cabanatuan City, Nueva Ecija Campuses. Founded in 1967, the university is one of the premier educational institutions in the country offering a wide range of academic courses and specializing in the medical and healthcare degree programs. It is a non-sectarian institution granted autonomous status by the commission on higher education (CHED). Our degree programs are accredited by the Philippine Association of colleges and universities.   • UNDERGRADUATE/BACHELOR’S DEGREE PROGRAMS

    Requirements

    Freshmen

    During Application:
    High School card not necessary.  Submit the accomplished Application Form to the Admissions Office.  Pay processing fee of P220, medical exam fee of P500 (for all programs except for Maritime Education) and the minimum fee of P1,500.

    To Validate Enrollment:
    After high school graduation, submit

    • Original High School Card, F-138
    • Original copy of Certificate of Good Moral Character
    • 3 Identical Photos, 2x2
    • Photocopy each of Diploma and PSA (NSO) Birth Certificate

    Transferees/SecondCoursers

    For Evaluation (credits subjects from previous school), submit any one of the following documents:
    • Scholastic Records;
    • Certificate of Grades;
    • Class Cards

    Requirements for Enrollment:
    • Honorable Dismissal/Transcript of Records or Transfer Credentials
    • Two (2) recent identical studio photos with white background
    • Clear photocopy of PSA (NSO) Birth Certificate

    Procedures

    Freshmen


    1.  Accomplish the Application Form and ;
    • Admissions Staff issues Student No. 
    • Pay the application processing fee of P220.00 at the Cashier’s Office
    • Secure the schedule for the FREE Scholarship Exam (optional).
     
    2.  Submit the following requirements:
    • Original High School Card or Form 138-A
    • Original Certificate of Good Moral Character
    • Three (3) copies-recent identical 2”x2” studio photo, white background
    • Clear photocopy of PSA birth certificate and HS diploma
    • Clear photocopy of Marriage Certificate (for married female applicants)

    3.  Pay the medical exam fee of 
    PhP 500 for all courses, except Maritime Education. PhP 900 for Maritime applicants.  


    4.  Interview with Admissions Officer to include the signing of Statement of Commitment and Conformity; Promissory Note and Waiver
     re:  Documents (for incomplete requirements); Discuss also payment plan:  Full /cash or downpayment plus 2 or 3 installments.


    5.  Payment of Fees:  Cash payment is accepted at the Accounting Office or designated Admissions Office Cashier and designated banks in Marulas, Valenzuela:  Unionbank, Metrobank, PNB, Eastwestbank, BPI, and Chinabank; Malinta: Landbank and Asia United Bank. For personal check payment,  proceed to the Student Accounting Office.   On the back of the check, please write:  (a)  Name of Student (b) Student No.  (c) College Program  (d) Home Address & Contact Numbers (landline and/or mobile phones)

    6. Present Payment Receipt to the Admissions Office Computer Encoder and claim Registration Card and Class Schedule upon submission of  all requirements (No. 2.)
     
    7. Finally, proceed to the Admissions Office Validator.  Claim the admissions kit, sketch of college uniform and redeem Credit Memo for scholarship discount, if any, immediately after enrollment or not later than June 30 or November 30 of the school semester.


    Transferees/Second Courses



    1.  Accomplish the Application Form.  
    • Admissions Staff issues Student No. 
    • Pay the application processing fee of P220.00 at the Cashier’s Office.
     
    2.  For evaluation (credit subjects from previous school/s),  submit any one of the following documents:
     
    • Scholastic Records 
    • Certificate of Grades  
    • Class Cards  
    • Transcript of Records
     
    The evaluation sheet is signed by both the Evaluator and Applicant.  Original copy remains in the student envelope; carbon copy to the applicant.
     
    3.  Pay the medical exam fee: 
    (a)  P500 for all courses, except Maritime Education. Proceed to the Fatima University Medical Center for (1) X-ray – 1st flr and (2) Laboratory – 2nd flr.  Results of both will be forwarded to the school physician who will notify only applicants with negative results for medication.
    (b)  P900 for Maritime applicants.  Proceed immediately to Ophtha and ENT clinics – 2nd flr – Fatima University Medical Center.  Applicant waits for results of color perception test and audiometry which must be submitted to the Admissions Officer; then to X-ray – 1st flr & Lab – 2nd flr.
     
    4.  Requirements for Enrollment:
    • Honorable Dismissal / Transcript of Records or Transfer Credentials
    • One (1) copy 2’ x 2” most recent photo (white background)
    • Clear photocopy of PSA / NSO birth  certificate
    • For married female, clear photocopy of Marriage Certificate

  • SENIOR HIGH SCHOOL PROGRAMS

    Requirements

    • Original Grade 10 Report Card (F-138)
    • Original Certificate of Completion of Junior High School
    • Original Certificate of Good Moral Character
    • 2 x 2 studio photo, white background (2 copies)
    • PSA/NSO Birth Certificate, 1 photocopy
    • Photocopy of Grade 10 Report Card (any quarter at time of application)
    • Qualified Voucher Recipient (QVR) Certifcate / Original ESC Certificate

    Procedures
    1.  Accomplish the Application Form and pay one-time fee of P720.00 to cover the: Medical exam, X-ray & Lab tests, and Application processing fee

      • School where Bachelor Degree was earned for those enrolling in the Master’s Program and School where MA was earned for those enrolling in the Doctor’s Program
    • Certificate of Employer if Currently Employed
