AMA University Online Education Requirements

New student applicants

1. Click Register button in the menu bar.
2. To register, fill up the online form and click Register button.
3. To continue your enrollment, open http://portal.amauonline.com, enter your email address and password then click Login button.
4. In the notification block, You may enroll in short courses by clicking Here button. Choose course to enroll.

Registered users

1. Open http://portal.amauonline.com, enter your email address and password then click Login button.
2. In the notification block, You may enroll in short courses by clicking Here button.

The following requirements must also be uploaded, these are scanned copy in PDF, DOC, JPEG, PNG format with no greater than 5MB file size:

• Transcript of Records
• Diploma
• Birth Certificate

Procedures

1. Go to www.amauonline.com
2. Click Register
3. Fill in all necessary information and Click the “Register” button at the end of the form. (The information your provide on the Registration form shall be recorded on your Registrar’s Student Portfolio therefore, make sure to complete the form before submitting ).
4. After successful registration, you will be redirected to “My Portal”. The Portal will require you to choose the academic program you want to enroll in. Upload the required documents in the following file formats: JPEG (.jpeg), TIFF (.tif), PDF (.pdf), PNG (.png), DOC (.doc)
5. Click submit for evaluation at the end of the registration application form. Requirements to be submitted will vary according to the applicants status upon registration.
6. Any case where documents are needed to be re-uploaded, the scanned documents can be uploaded to [email protected]

K11 Student Applicants:

• Successful Online Registration;
• Form 138/ Grade 10 Card;
• Certificate of Completion;
• Certificate of Good Moral;
• NSO Certified Birth Certificate;
• Form 137 or the Transcript of Records (School-toSchool Transaction)

Transferees:

• Successful Online Registration;
• K11 Form 138 or Grade 11 Card;
• NSO Certified Birth Certificate;
• Form 137 or the Transcript of Records (School to School Transaction)

Freshmen

• Accomplished Online Application Form
• High School Report Card (Form 138)
• High School Transcript (Form 137A) Note: This should be a school to school transaction to ensure its authenticity
• Certificate of Good Moral Character
• 2x2 Photo (for 201 file)

ALS Graduate

• Certified Copy of the Diploma
• Original copy of Birth Certificate

Transferees

• Accomplished Online Application Form
• Honorable Dismissal/ Transfer Credentials
• Certificate of Good Moral Character
• Official Transcript of Records to be sent by the previous school with anotation: COPY FOR: AMA Building II, Panay Ave., Quezon City with subject description taken from previous school (if needed for crediting of subjects)
• 2x2 Photo (for 201 file)
• Original copy of Birth Certificate or legal adoption papers.

Transferees from AMAES Member Schools

• Accomplished Online Application Form
• Letter of Intent to Transfer
• Honorable dismissal from originating AMAES School Official Transcript of Records with notation: COPY FOR: AMA University, Project 8, Quezon City
• Certificate of Good Moral Character
• 2x2 Photo (for 201 file)
• Original copy of Birth Certificate or legal adoption papers

Second Course and Graduate Students

• Transcript of Records from last school attended
• Diploma
• Birth Certificate or Certificate of Live Birth (NSO Certified Copy)

Foreign Students

• Accomplished Online Application Form
• Photocopy of Student Visa
• Letter of Consent approved by the Commission on Higher Education (CHED)
• Five copies of revised original Personal History Statement (PHS) duly accomplished and signed by the applicant in English and in national alphabet, accompanies by personal seal, if any, original left and right hand prints on PHS and original photos
• A notarized affidavit or support and proof of adequate financial support to cover student’s dues and other incidental expenses.
• For scholars, Certificate of Scholarship from the institution concerned
• Photocopy of passport pages where name, photo, birth date, and birth place appear
• Photocopy of Birth Certificate of legal adoption papers
• Two copies of (1x1) photos (items 1-4 needed for student visa application)
